To complete your form two pieces of information need to be uploaded as listed below, plus additional sections to be completed:

• Passport size photo.
• Full list of subjects taken with grades and subject you intend to take..
• Choice of University.
• Colleges choices.
• Why Medicine? For this and other sections where text is inputted, it might be more convenient to write the text in Microsoft Word or similar and then paste your answers into the text box. 

We appreciate that we are asking for a great deal of information, but this is a bespoke course that will be personalised just for you. The more information that we have the better able we are to get the best results for you.
